Service Description

Initial clinical assessment of presenting symptoms for diagnosis, history gathering, and treatment planning. Will require completion of intake paperwork, which includes office policies, privacy policy, treatment consent, financial agreement, and history background.

Cancellation Policy

Please give a 24-hour advance notice if you must cancel your appointment. A fee of $100 is charged if an appointment is missed or cancelled with less than 24-hour notice, unless due to illness or an emergency.
